The Freedom of Information Act (FOIA) governs the inspection of public records. The Act requires that all public bodies make their public records available, with the exception of specific exemptions.
To request public records from the Village, follow the Freedom of Information Request Form link below. The link will take you to the online FOIA portal. Choose the appropriate FOIA category, fill out the fields completely and submit. It will be routed to the Freedom of Information Act (FOIA) Officer. Village staff will reply within five days.

Fire and EMS Records
Ambulance Billing
The Fire Department utilizes a third-party agency, Andres Medical Billing, for its ambulance billing. If you have questions about an ambulance bill, or would like to obtain a copy of the bill, please contact Andres Medical Billing.
Ambulance Reports
The Fire Department’s ambulance reports/medical records are processed and stored with the Northwest Community Emergency Medical Services System. If you would like to obtain a copy of an ambulance report/medical record, please contact Sarah Gonet at 847-870-5660, or sgonet@mountprospect.org.

The Fire Department maintains records for all fire investigations. If you would like to inspect or receive copies of fire investigation reports, you must request the information through the Freedom of Information Act (FOIA).
