The HDHP features a nationwide network of designated medical providers. Participants may NOT choose physicians and medical providers outside the Preferred Provider Organization (PPO) listing. This plan has an annual deductible and an annual out-of-pocket maximum.

Terms to know under the Blue Cross Shield Illinois PPO Plans:
- Deductible is the amount of covered expenses you or your covered dependent incur during the plan year before the plan will pay benefits.
- Co-payment is the percentage the employee pays after the deductible has been met.
- Out-of-pocket limit is the maximum amount you may pay each year for covered expenses. After this limit is met, the plan will pay 100% of eligible expenses. (Note: Any additional expenses over and above the out-of-pocket-maximum may result when using a non-PPO provider and/or the expenses are considered above the normal and customary expenses.)
- Normal and Customary Expenses describes those maximum allowable charges for a treatment, supply, or service, which fall within the usual range of charges for the same service or supply in a geographical area.

BCBS Global Core - Eligible BCBS members can utilize this website to find doctors and hospitals outside of the United States, Puerto Rico and U.S. Virgin Islands. Member can also find other valuable resources to help stay safe and healthy around the world.

Blue Cross Blue Shield (BCBS) offers two (2) vision discount programs through Davis Vision and EyeMed for PPO Participants.
