In-Home Assessment
In-Home Assessment: Homebound seniors, who are finding it difficult to remain in their homes due to various circumstances, can receive a free confidential assessment in their home with the Senior Services Social Worker and the Public Health Nurse. Qualifying circumstances include, but are not limited to, decreased mobility, medication management problems, trouble monitoring chronic illnesses like diabetes or hypertension, transportation issues, meal preparation and those with limited or no support. Recommendations and resources will be provided.
