Clinic Services
- Screening Clinic: Our nurse sees patients for free blood pressure checks, heart rate, oxygen level, and blood sugar (glucose) checks, and $2 total cholesterol screenings every Tuesday morning between 9:00 a.m. and 11:00 a.m. in the Nurse's Office on the 2nd floor at Village Hall. Appointments are required. Please call 847-870-5680 to schedule an appointment.
- A1C Testing: $20 test that can be paid via cash or check. An appointment is required. No fasting is required. The results will indicate the average amount of sugar in your blood over the past few months. Results are ready in 8 minutes.
- Lipid Panel: $20 that can be paid via cash or check. An appointment and 8 hour fast is required. The test yields 6 results, including total cholesterol (TC), HDL (good cholesterol), LDL (bad cholesterol), triglycerides, non-HDL, and TC:HDL ratio. Results are ready in 5 minutes.
Other Health Services
- RN Office Visit: Appointments can be scheduled with the nurse for individual consultations on health-related concerns such as medication counseling, diet & nutrition, health education, etc.
- Memory Screenings:
The Village Nurse offers FREE, confidential memory screenings by appointment only. The screenings are part of an initiative of the Alzheimer's Foundation of America designed to promote proper detection of memory problems. While the screening results are not a diagnosis, they can help identify any memory issues to be addressed with a physician. Must be a Mount Prospect resident. In Home Assessment: Homebound seniors, who are finding it difficult to remain in their homes due to various circumstances, can receive a free private assessment in their home with the Senior Services Social Worker and the Public Health Nurse. Qualifying circumstances include, but are not limited to, decreased mobility, medication management problems, trouble monitoring chronic illnesses like diabetes or hypertension, transportation issues, meal preparation and family members living far away. Recommendations and resources will be provided.

Information and Referral: Residents are provided with health information, resources and referrals as appropriate.
- Health Education Classes and Seminars: The nurse develops classes and seminars, or brings in experts on a wide range of topics of general interest . Past topics have included Cholesterol, Blood Pressure, Healthy Heart, Planning for Care, Memory Loss, etc.
- Food Pantry Health Programming: Food pantry clients are eligible to meet with the Nurse to evaluate their diet and the impact it has on their health. The nurse posts monthly educational materials including recipes and healthy lifestyle tips for food pantry clients.

File of Life Program: When there’s an emergency, every second counts. Emergency First Responders will now be able to reference important medical information for a patient in the event of an emergency. The File of Life allows residents to document vital information in one place, including emergency contacts, Living Will information, medical conditions, allergies, and medications. This program offers benefits to both residents and first responders. It allows first responders to instantly know the medical history of a patient, which allows for faster help and corrective treatment. The benefits to residents include peace of mind knowing they will have prompt and quality care, easy access to potentially life-saving information, and assurance that proper persons will be notified. Please contact the Human Services Department to pick up a File of Life card and refrigerator magnet pouch.

Premise Alert Program: The Nurse assists with Village’s Premise Alert Program. The Premise Alert Program consists of a database of individuals with special needs or disabilities stored in a computer aided dispatch (CAD) database maintained by Northwest Central Dispatch System (NWCDS.) NWCDS provides dispatch service for Mount Prospect police and fire personnel. The Program will assist police and fire personnel to identify individuals with special needs or disabilities, which will enable Mount Prospect police and fire personnel to have additional information at their disposal when responding for emergency services.

- Aging in Community Initiative: Health and wellness has been identified as a priority in the Aging in Community Initiative. The Health and Wellness Workgroup will implement goals and objectives identified in the Aging in Community Action Plan. The Village Nurse co-chairs the Health and Wellness Workgroup.

The Medical Lending Closet is located at Village Hall, on the 2nd floor in the Human Services Department. Items are loaned to residents at no charge for up to three months, excluding wheelchairs which are loaned for 1 month. Loans are not meant to be permanent arrangements. Medical equipment is available on a first-come, first-served basis. Residents will be asked to sign a waiver form when receiving donated items. The lending closet is open on Mondays, Wednesdays and Thursdays from 8:30 a.m. to 4:00 p.m. Please call 847-870-5680 to schedule a pickup.

Not Accepted
For health, hygiene, and liability reasons, we do not accept or distribute the following:
- Toileting Items: Commodes and Raised Toilet Seats
- Single-Use/Soft Goods: Braces, slings, or cushions
- Broken or Modified Equipment: Any items with missing parts, rust, or structural damage.
- Large/Electric Equipment: We do not accept electric wheelchairs or hospital beds.
- Medications
