The Freedom of Information Act (FOIA) governs the inspection of public records. The Act requires that all public bodies make their public records available, with the exception of specific exemptions.

To request public records from the Village, please download the following Freedom of Information Request form, complete it in its entirety, and submit it to the Mount Prospect Village Clerk. Forms can either be faxed (847/392-6954) or mailed to 50 South Emerson Street, Mount Prospect, Illinois 60056-3266. Village staff will reply within seven days.
