COMMUNITY HEALTH SERVICES
- Annual Community Blood Test – An annual blood test is offered to all
residents every September at a nominal cost. It consists of a complete blood
analysis including HDL, LDL, and cholesterol.
- Flu Shot Program -- Flu shots are administered annually in the fall, and are
especially appropriate for those who are over age sixty-five and other adults
who suffer chronic illnesses. Medicare claims are filed for senior citizens who
have Medicare Part B, excluding those enrolled in an HMO and Railroad
Retirement. HMO and Railroad Retirement members and all other adults receive the
inoculation for a nominal fee.
- Medical Equipment Lending Closet -- Medical equipment is located at the
Human Services Department and is loaned to residents at no charge. Equipment
includes walkers, wheelchairs, crutches, commodes, etc.
- Physician Advocacy -- Sometimes, due to the stress of being in the doctor’s
office or hearing about a new diagnosis, it is difficult for patients to ask
questions or to fully understand what their physician is saying. The nurse is
available to discuss a full range of medical issues with Mount Prospect resident
and their physicians.
HEALTH SCREENINGS -- IN THE OFFICE
- Blood Pressure Clinic -- Our nurse sees patients for blood pressure check
and diabetic testing every Monday morning between 9:00 a.m. and 12:00 noon. No
appointments are necessary.
- Cholesterol Screening -- Cholesterol testing is performed on the second
Thursday of every month for a nominal fee. The machine we use is the latest
model that offers very accurate readings of the total Lipid Panel, which
consists of total cholesterol and the HDL, LDL, Triglycerides and VLDL.
- Health Education Classes and Seminars -- The nurse develops classes and
seminars on a wide range of topics of general interest. Some topics have
included Cholesterol, Blood Pressure, Healthy Heart, Planning for Care, etc.
- Monthly Health Screenings -- Throughout the year, Human Services sponsors
various health screenings, such as hearing, heart and stroke, bone density,
cancer, etc.
- RN Office Visit -- Appointments can be scheduled with the nurse for
individual consultations on health related concerns such as medication
counseling, wound care, health education, etc.
HEALTH SCREENINGS -- OUTREACH
- Community Outreach Nursing -- Our nurse provides skilled nursing care to
homebound residents who fall between the gaps of existing, insurance
reimbursable, services. This service has allowed many residents the option to
remain in their homes.
- Community Partnership Immunization Clinic -- This monthly, cooperative
effort involves Mount Prospect, Arlington Heights and Elk Grove Village nurses,
Northwest Community Hospital and Cook County Department of Public Health, and
provides easily accessible immunizations to residents in the Northwest suburban
area.
- Senior Subsidized Residence Health Services -- The nurse makes regularly
scheduled visits to offer health screening services and flu shots to residents
at these specific off site locations.
HOME SAFETY AND SERVICES
- Call Check -- This program is designed to provide telephone support to frail
residents who do not have the physical or mental resources and family to provide
such support during times of wide spread, extreme weather conditions with power
outages.
- Home Delivered Meals -- This program provides our homebound and frail
residents, who live alone and are over the age of sixty, with a hot nutritious
meal. Assessments are made by the social worker and nurse to determine
eligibility and to ensure the individual is safe.
- LifeLine Program -- This program, which was developed by the Fire and Human
Services Departments, provides vital information when you can not speak for
yourself. In emergency situations, the paramedics look for the red envelope
affixed to the outside of the refrigerator. The envelope contains important
personal, medical and insurance information about you.
