| Call Check and Lifeline Programs |

The Village has two programs that may be of great benefit to our frail
residents: The Call Check Program and the Lifeline Program.
The Call Check Program provides support for residents in the form of telephone
reassurance, advocacy, and referrals to appropriate parties. It is intended for
frail individuals who do not have the physical or mental resources; or family
available, to provide such support.
This program will go into effect in times of extreme weather conditions,
including heat indexes of over 105 degrees; a combination of extreme cold and
ice; extreme temperatures and power outages; heavy winds and power outages.
Note: Conditions must be widespread so that the Village is aware of the problem.
Under this program, when Village of Mount Prospect staff feels that the weather
conditions warrant it, participants of the Call Check Program will be contacted
by Human Services personnel.
The Human Services and Fire Departments developed the LifeLine Program to ensure
that when an emergency arises, the vital information needed by the paramedics is
ready and available. LifeLine forms indicate an individual’s name, address,
doctor’s name, medications, medical history, allergies and emergency contact
persons. After completion, the form is enclosed in a special red envelope that
can be placed in a conspicuous place on the front of the refrigerator.
Information indicated on this form can be of great assistance to emergency
personnel. Mount Prospect paramedics are familiar with the envelope and know to
look for it. Forms and assistance in filling them out are available from the
